Annual Fees:

$ 2,000 / year: Individual age 40 and over as of 10/1/11

$ 3,800 / year: Adult couple age 40 and over as of 10/1/11

$ 1,500 / year: Individual age 39 and under as of 10/1/11

$ 2,800 / year: Adult couple age 39 and under as of 10/1/11

$ 500 / year: individual age 18-26 as of 10/1/11

NO CHARGE for dependent children of a member if living at home or current student under 23 years old.

The annual fee is paid directly to Norcross Family Medicine and is not covered by your insurance.  You will need to keep your insurance as the annual fee only covers services provided by Dr. Tim or Dr. Gwenne Norcross.  You will need your insurance to cover lab tests, specialists, hospitalizations and other medical services.
